It was initially formed in the Kharkov Military District. With 6th Rifle Corps, 6th Army of the Southwestern Front from 22 June 1941. Wiped out at Kiev September 1941. Recreated at Chapayevsk March 1942, wiped out at Izyum May 1942. Recreated again in October 1942 at Verchovye from 118th Rifle Brigade, fought at Kursk and in Poland.
