词条 | Comprehensive medication management |
释义 |
CMM is generally delivered directly by a pharmacist in a clinic setting, in collaboration with other health care providers including primary care providers, nurse care coordinators, social workers, dietitians, diabetes educators, behavioral health, and more. Pharmacists who conduct CMM generally have a collaborative practice agreement with a physician at their site of practice, allowing them to prescribe and adjust medications for several chronic conditions including high blood pressure, diabetes, high cholesterol, asthma, chronic-obstructive pulmonary disease, anticoagulation management and smoking cessation among others.[6][7] Beyond assessing a patient’s medications in their present state, the pharmacist delivering CMM will work with the patient to develop goals for the utilization of drug therapy, and schedule continual follow-up to ensure these goals are met.[4][6] A key component of CMM is patient-centeredness, referring to the process by which the patient understands and agrees to the goals of therapy and actively participates in the plan for care.[5][6] References1. ^{{Cite web |url=https://innovations.ahrq.gov/qualitytools/patient-centered-medical-home-resource-guide-integrating-comprehensive-medication |title=The Patient-Centered Medical Home: A Resource Guide for Integrating Comprehensive Medication Management to Optimize Patient Outcomes (Second Edition) |website=innovations.ahrq.gov |publisher=AHRQ Health Care Innovations Exchange, Government of United States |location=United States |language=en-US |publication-place=United States |access-date=2019-02-12}} {{Med-stub}}2. ^{{Cite journal |title=The Philosophy of Practice for Comprehensive Medication Management: Evaluating Its Meaning and Application by Practitioners |journal = Pharmacotherapy: The Journal of Human Pharmacology and Drug Therapy|volume = 38|issue = 1|last=Pestka |first=Deborah L. |last2=Sorge |first2=Lindsay A. |date=January 2018 |publisher=National Center for Biotechnology Information, U.S. National Library of Medicine |location=US |pages=69–79 |publication-place=US |doi=10.1002/phar.2062 |pmid=29155443 |last3=McClurg |first3=Mary Roth |last4=Sorensen |first4=Todd D.}} 3. ^{{Cite journal |title=Comprehensive Medication Management and Medication Adherence for Chronic Conditions |journal = Journal of Managed Care & Specialty Pharmacy|volume = 22|issue = 1|last=Brummel |first=Amanda |last2=Carlson |first2=Angeline M. |date=January 2016 |publisher=National Center for Biotechnology Information, U.S. National Library of Medicine |location=US |pages=56–62 |language=en-US |publication-place=US |doi=10.18553/jmcp.2016.22.1.56 |pmid=27015052 }} 4. ^1 {{cite web |url=https://www.pcpcc.org/sites/default/files/media/medmanagement.pdf |website=Patient-Centered Primary Care Collaborative |title=The Patient-Centered Medical Home: Integrating Comprehensive Medication Management to Optimize Patient Outcomes RESOURCE GUIDE}} 5. ^1 {{cite web |url=https://www.accp.com/docs/positions/misc/CMM_Care_Process.pdf |website=American College of Clinical Pharmacy |title=The Patient Care Process for Delivering Comprehensive Medication Management (CMM)}} 6. ^1 2 {{cite web |url=https://www.accp.com/docs/positions/whitePapers/CDTM%20CMM%202015%20Final.pdf |website=American College of Clinical Pharmacy |title=Collaborative Drug Therapy Management and Comprehensive Medication Management―2015}} 7. ^{{cite web |url=https://www.powerpak.com/course/print/113185 |website=Power-Pak Continuing Education |title=Module 1.
