The Shade Tree Clinic is a free health clinic located in Nashville, Tennessee. Established in 2004 by students at Vanderbilt University School of Medicine (VUSM), the clinic provides free medical care and legal assistance to uninsured individuals.[1][2] The clinic, situated in East Nashville, currently serves around 400 patients each year, including many who do not speak English.[1][3] Shade Tree Clinic is staffed by student volunteers from VUSM, Vanderbilt University School of Nursing, Vanderbilt University Law School, and the University of Tennessee School of Pharmacy under the supervision of licensed Vanderbilt physicians.[5][1] The Shade Tree Clinic is a classified as a student-run free clinic (SRFC), one of approximately 208 in the nation.[4]

History

The Shade Tree Clinic was started in 2004 by medical students at VUSM as a free walk-in clinic.[3] The clinic was initially very small, operating out of a double-wide trailer, though it has since moved to a permanent building in East Nashville.[5][6] At that time, the clinic only offered limited outpatient care, primarily for acute issues, such as for birth control and urgent care needs.[3] Over time, however, a greater proportion of the patients using the clinic were receiving treatment for chronic illness. As of 2018, two-thirds of the people using the Shade Tree Clinic have one or more chronic disease, such as diabetes, hypertension, or chronic obstructive pulmonary disease.[3]

Through the years, the clinic has also made several expansions to the types of medical care it offers to patients. In 2009, the Shade Tree Clinic implemented a new program to provide prenatal care to pregnant women. This service, called the Shade Tree Early Pregnancy Program (STEPP), aims to provide newly pregnant patients with a comprehensive prenatal visit at the clinic before providing a bridge to continuing prenatal care through the Vanderbilt University School of Nursing.[7] Beginning in 2017, the clinic also established a program with Vanderbilt University Medical Center (VUMC) to provide free surgical care for some patients. In the first year of this initiative, volunteers at VUMC gave surgical treatment to nine patients with hand conditions such as carpal tunnel syndrome and trigger finger.[8]

While initially only providing health care, in 2012 the Shade Tree Clinic moved to start providing free legal assistance to its patients as well.[9] The new program was born out of a medical-legal partnership between the Shade Tree Clinic and the Legal Aid Society of Middle Tennessee and the Cumberlands.[9] The addition of this service was prompted by an unmet need to address some of the social determinants of health that could not be solved by medical care alone.[1]

To sustain the medical, legal, and social services it provides, the Shade Tree Clinic hosts two annual fundraising events: the [https://shadetreeclinic.org/events/dinner-auction/ Shade Tree Benefit Dinner and Silent Auction] and the [https://shadetreeclinic.org/events/shade-tree-trot/ Shade Tree Trot], a 5K race.[10]

As of 2017, the Shade Tree Clinic had provided care for more than 4,000 patients.[5]

Mission

The mission of the clinic, as described in the Shade Tree Clinic 2016-2017 Annual Report, is:

"We strive to be the primary care home for underserved individuals in Davidson County by providing comprehensive care for chronic illnesses, social services, and health education. To meet this mission, STC forms interdisciplinary teams of medical, nursing, pharmacy, and law students who are supervised by professionals in these fields to provide healthcare and social services free-of-charge to this community. The clinic welcomes all patients, regardless of their race, residency, gender, socioeconomic status, or religious affiliation."[5]

Scope of Services Provided

The services offered by the Shade Tree Clinic fall into three main categories: medical, legal, and social services.

Medical Services

The clinic, which operates by appointment on Tuesdays and Saturdays, serves as a primary care clinic for its approximately 400 patients.[5] Throughout the year, the Shade Tree Clinic also hosts specialty clinics that provide a number of different types of specialized medical care, supervised by more than 46 licensed medical providers.[3]

The Shade Tree Clinic is one of just several SRFCs in the nation that has a fully functional dispensary onsite. This service, which operates at all specialty and primary-care clinics, is able to provide patients with the vast majority of their free medications at the time of their clinic visit.[5][11] The medication dispensary carries over 200 different types of medications, although it does not carry narcotics, depressants, hallucinogens, or medications that are potentially addictive or psychoactive.[11] Students from the University of Tennessee College of Pharmacy join medical students from VUSM in running the dispensary.[11]

The clinic also features a fully functioning clinical laboratory. This onsite office, which operates at all specialty and primary-care clinics, does phlebotomy and point-of-care testing. The laboratory services team also provides vaccinations.[5]

Social and Legal Services

A number of social services are provided at the Shade Tree Clinic, some of which are aimed at addressing social determinants of health and others of which are meant to facilitate the delivery of care.

The health educator program at the clinic places a first-year medical student, called a Patient Health Educator (PHE), with a Shade Tree patient with a chronic health condition. The PHEs provide health counseling and facilitate year-long continuity of care by following patients' lab results.[12] A 2014 study of the care and outcomes of patients with diabetes seen at Shade Tree found that a medical student health educator program in the context of a SRFC was an important part of providing high-quality care leading to clinical improvement in these patients.[13]

Several services, including legal aid provided via the Legal Aid Society of Middle Tennessee and the Cumberlands, Spanish interpretation, and social work are present at every clinic.[5][9] Additionally, the clinic has a Pharmacy Assistance Program that allows patients to apply for free or reduced-cost medications. Among many other drugs, the Shade Tree Clinic has been able to provide patients with Hepatitis C with the drug Harvoni, which normally costs $90,000 for a full treatment but can cure hepatitis in up to 90% of patients.[5]
