1. The Form

Standard Form 52 (SF 52), officially titled Request for Personnel Action, is a United States government form used to initiate various personnel actions for government employees.

The SF 52 itself is not the action, it only begins the process. The Standard Form 50 (SF 50, "Notification of Personnel Action") is the official document which shows what action was taken on behalf of the employee.

The Form

The SF 52 is divided into six Parts (A through F).

Part A contains six blocks, which state the action requested (1), the request number (2), contact information (3), the proposed effective date of the action (4), and the requestor's and authorizer's information (5 and 6).

Part B contains 51 blocks. The first 44 blocks mirror exactly the first 44 blocks of the SF 50, so whatever is on the SF 52 will ultimately flow to the SF 50. The remaining seven blocks contain information which may or may not be on the SF 50: educational level (45), year degree obtained (46), academic discipline (47), functional class (48), citizenship (49), veteran's status (50), and supervisory status (51). Part F (on the back of the second page) allows for remarks to be added, which will ultimately flow to Block 45 of the SF 50.

Part C lists places for various reviews (Block 1 and sub-blocks), and approval (Block 2).

Parts D and E are used when an employee decides to leave civil service (either by resignation or retirement). Part E has five blocks: reason for departure (1, this is voluntary and an employee need not give any reason for leaving, but it may be considered should the employee wish to return to civil service), effective date (2, the effective time is midnight unless specified otherwise), signature and date (3 and 4), and a forwarding address (5). Part D is a yes/no series of boxes to be used by a supervisor; if there are additional or conflicting reasons for departure the reasons are to be included on a separate sheet attached to the SF 52.
